22 y/o male with a self-palpated painless testicular mass, normal serum tumor markers, and an 11mm heterogeneous, hypoechoic, well-marginated vascular mass on US. Right orchiectomy showed a firm, white-tan, well-circumscribed, 1.2 cm lower-pole tumor abutting the tunica albuginea (H&E images 1-4). IHC was positive for β-catenin (image 5) and vimentin, but negative for CK-OSCAR, AE1/AE3, EMA, CK7, CK20, inhibin, calretinin, WT-1, Melan-A, PLAP, SALL4, AFP, CD117, CD30, GATA3, p63, desmin, ERG, NKX3.1, and Ki-67 (1%).
